The goal of home visiting nurse program is to improve the health of women, children and their families. Visits begin during the first or second trimester of pregnancy and continue until the child’s second birthday. Referrals must be received before 28 weeks gestation. They offer help with health insurance, connection with other resources, including WIC, counseling, legal and other support services. Client education focuses on promoting healthy lifestyle choices and nurturing parenting skills.

 
Maternal Child Health Field Nurses

Home visits utilizing evidence-based programs serving low-income, first-time mothers and mothers parenting a child under the age of 5 who have medical and/or social risk factors. Priority is given to clients identified as having medical and/or social risk factors that put the pregnancy in jeopardy or interfere with effective parenting.

 
Teen Parent Connection
Mahiri, Kem, TPC Supervisor

Teen parenting is a program for pregnant or parenting teenagers age 18 or younger. Long term comprehensive case management services to maximize educational potential and support self-sufficiency.
